Lisa LaChapelle, RN, CCM, is the Transitional Care Program Case Manager for Johns Hopkins Healthcare. Johns Hopkins Healthcare provides care for Medicaid recipients, military dependents, and retired military through a Department of Defense contract. Mrs. LaChapelle is an RN and CCM who has always wanted to work with the military family in a Case Management / Care Coordination position. She is the wife of a retired NCO, and is the proud stepmother of a Lieutenant in the Unites States Navy.
She received her LPN from South Central Area Vo-Technical School in 1972, and her A.D.N. from Harford Community College. She is currently enrolled in a BSN program at the University of Phoenix. Lisa is a Certified Case Manager with 10+ years of experience in both case management and disease management. Her clinical experience is in Geriatrics, Orthopedics and Medical – Surgical Nursing. She has successfully functioned in roles varying from case manager, to utilization review nurse, to home health nurse, to staff nurse.
Mrs. LaChapelle’s presentation will be on a subject that is very near to her heart--working with military clients as they transition thru the healthcare continuum.
