Kathleen Grieve RN, BSN, MHA, CCM, joined John Hopkins HealthCare in January 2003 as a Disease Case Manager in the areas of cardiovascular disease, asthma, and diabetes. In her current role,she is a Guided Care Nurse in the John Hopkins HealthCare Guided Care Model in which she partners with three to five primary care physicians to care for their most complex, high-risk, older outpatients to improve quality of healthcare, quality of life, and efficiency of healthcare.
Her previous work experience includes staff nurse in the neurotrauma unit at the Shock Trauma Center at the University of MD Medical Center and various positions with the Maryland Correctional Facilities. She also has experience in worker compensation cases throughout the State of Maryland, Northern Virginia, and Washington, DC. For two summers, she has taken mission trips to Haiti and Guatemala to assess and treat ambulatory patients in eight different villages.
