Pamela Jean Tobichuk, RN, BSN, received her RN diploma from Brockton Hospital School of Nursing in Brockton Ma and her Bachelor of Science in Nursing from Regis College in Weston Ma. She holds ONC certification (Orthopedic Nurse Certified). In her current role at MGH she is one of two Case Managers who manage the Orthopedic Total Joint Preadmission Program for total hip and knee replacement patients administering the Risk Assessment and Prediction Tool(RAPT) to develop comprehensive discharge plans.
Her other responsibilities include Emergency Room Case Management in the evening where she provides support and education to medical staff on the most appropriate setting and level of care for patients, setting up discharge services as needed, and problem solving after hour case management issues for the entire hospital.
Pam has presented to her colleagues on utilization and outcomes of the RAPT. Pam participates in the Orthopedic Clinical Care Services(CPM) team and promotes and educates orthopedic leadership on process improvements for the RAPT and Orthopedic Total Joint Program.
She was the president of her local chapter of NAON(National Association of Orthopedic Nurses) for four years and has organized many educational offerings for her chapter members and remains an active member. She has also recently joined CMSA.
