Lisa M Rivera, RN, BSN, PHN             Team Lead Transplant Anthem Woodland Hills, CA             Lisa Marie Rivera, RN, BSN, PHN, is currently the Case Management/Utilization Management Team Lead for the Western Region of Anthem Blue Cross Centers of Medical Excellence - Transplants. She is responsible for many functions, but most importantly for directly supporting the Transplant Care Management associates to effectively and efficiently implement the case management process, throughout the continuum of care, using direct contact and collaboration with the member/family, primary physician, and other multidisciplinary team members. Lisa received her nursing degree from San Francisco State University, and graduated with honors – magna cum laude. She started her career in the home health setting, but had spent the majority of her profession as an innovator and leader in long term care facilities, as a Director of Nursing. She transitioned into Case Management in January 2007, and though her experience in this field is limited, she is a strong leader who is dedicated to achieving optimal care outcomes.
