| | | The Pre-Conference Program is offered at the 25 percent discounted fee of $971.25, which includes the comprehensive CCP program/reference manual, examination, certification, and continuing education credit for nurses.
Based on a very positive response to last year’s session, HealthSciences Institute will again offer the two-day Chronic Care Professional (CCP) National Certification Program. Since 2004, the CCP program has been selected by leading health plans, disease management organizations, state public health agencies, and health systems across the US and Canada.
The CCP program is the only evidence and competency-based disease management program and credential for case managers. CCP certification was recognized in 2007 by the Disease Management Association of America (DMAA) as the recommended credential for case managers who provide health and disease management services.
The CMSA CCP preconference session is a 40-hour blended program that combines a two-day live session with a comprehensive eLearning program offered through HealthSciences Institute’s University partner. Following the two-day session, participants complete the self-directed eLearning program and take the on-line certification exam at a later date.

This continuing nursing education activity was approved by the Wisconsin Nurses Association Continuing Education Approval Program Committee, an accredited approver by the America Nurses Credentialing Center’s Commission on Accreditation. The program has been approved for 40 contact hours for nurses.
