Following an acute myocardial infarction or other thrombotic event, patients remain at high risk for future coronary events. Therapeutic interventions initiated immediately following an acute episode and continued in the outpatient setting are key to successful patient outcomes.
In this symposium, case managers will learn about the importance of antithrombotic therapy in managing patients with non-ST segment elevation myocardial infarction (NSTEMI) and patients with ST-segment elevation myocardial infarction (STEMI). Recent clinical trials and national consensus treatment guidelines describing optimal management of these conditions will be reviewed with emphasis on topics important to case-managers - patient safety, quality of life, and cost-effectiveness.
Case management principles and strategies for incorporating evidenced-based quality improvement measures and treatment guidelines for the patient with Acute Coronary Syndrome (ACS) in the acute care setting and after discharge will be addressed.
Objectives:
- Explain the pathogenesis of Acute Coronary Syndrome (ACS), with an emphasis on the role of platelets, and cite key findings from the CRUSADE trial, to identify gaps as patients’ transition through the healthcare continuum.
- Discuss the use of antithrombotic therapy and other non-pharmacologic therapies as an important component of the transition of care plan in patients with a diagnosis of UA/NSTEMI or STEMI.
- Detail case management principles, interventions, and strategies that advance health behavioral change, adherence and medication persistence through every transition of care.
- Identify tools and strategies to implement evidence-based quality improvement measures and treatment guidelines to improve outcomes in patients with Acute Coronary Syndrome (ACS).
