Patients often experience a sense of loss and confusion as they transition from one healthcare setting to another. Providers often fail to communicate important information, medications, treatment plans, laboratory tests, consultations, and discharge instructions to the patient, caregiver, or the provider at the next level of care. This session will discuss the barriers and issues related to transitions of care, and offer solutions to improving the care from the various healthcare settings to the workplace.
Objectives:
1. Identify the barriers and their impact in transitions of care.
2. Assess tools and resources available to improve the transition process.
3. Evaluate performance metrics to support appropriate transitions. |
