In this session, you will learn how a Medicaid Managed Care Plan improved their HEDIS scores by using the following recipe:
- One cup of collaboration--with providers, vendors, leaders in the healthcare community, and the state Department of Health,
Add:
- A dash of "Supporting Excellence"--a Pay For Performance (P4P) program developed to support and encourage providers' efforts to deliver excellent services to their members
Sprinkle With:
- A tablespoon of "CMAG"--medication adherence guidelines developed by the Case Management Society of America
Mix well; bake for one year, and do a taste test.
In this didactic session, you will see examples of the actionable reports developed to assist providers in delivering care; learn the actual dollar investment (pmpm) by the plan and by the state; and see the results of these efforts. Join us and decide for yourself if Collaboration + P4P + CMAG is indeed a recipe for success!
Objectives:
- Identify three provider incentives used in the P4P program (Supporting Excellence).
- Determine what the ROI is for the Supporting Excellence Program.
- Identify four motivational inter-viewing techniques adopted into these programs from the medication adherence guidelines (CMAG).
- List the elements of a successful “actionable” provider report.
