As part of the Centers for Medicare and Medicaid (CMS) requirements for Medicare Part D, health care plans are required to provide Medication Therapy Management (MTM) programs. This is for Part D beneficiaries who meet certain criteria set forth in part by CMS. The criteria is used to assist in identifying those participants who have multiple chronic diseases and are at risk for medication-related problems.
One large health plan has developed an MTM program which collaborates with case management. This program includes client education, web-based health management, and comprehensive geriatric pharmacist reviews. In addition, issues related to polypharmacy, medication adherence, potentially inappropriate medications in the elderly, questionable narcotic utilization, renally adjusted medications, and potential underutilization of certain medications in diabetic patients are addressed.
This presentation will discuss the collaboration between the MTM program and case management as well as describing the basic program design, goals, and intervention strategies. Recommendations for future development and best practices will be included.
