Ingredients for successful Case Management come together like the ingredients in a fine cake. And like the recipe for a fine cake, the ingredients must be of the highest quality, carefully considered, measured and mixed at just the right time. As the architect of a wonderful cake, or a good Case Management Program, it helps to have a map, a recipe to bring all the right elements together and great tools to see the process through.

When Beacon looked at their Case Management Program in 2005, it was clear that we had the right people, good policies, procedures and abundant appreciation from members, however, we needed better tools to serve something really special to members, our clients and stakeholders. We knew that most importantly we needed better system support to reflect the good work being done by Case Managers.

At first, it seemed the language and goals of IT and Clinical Departments might be hard to bring together for a blended goal, but through collaborative discussion and development, together we developed a data base that brings together the important components of Case Management: referral, enrollment, assessment [include risk scores] problem identification, short and long term goals, care planning, case closure and assessment on closure through an interactive data base.

The enhanced system provides better documentation, improved reports, and measurements for clinical and utilization outcomes. Our presentation will focus on our journey, working together, the development and components of the Case Management Module that now monitors and tracks the various elements in our Case Management Program and has produced an organized and comprehensive path to follow.

We hope the presentation will be of interest to Case Managers that may be just starting this process, or to others that are preparing the icing for their own fine cake.

Attendees will understand the process steps involved in mapping out goals, actions steps, and timelines for Case Management system development

Attendees will be clear regarding key elements and functionality that should be present in an electronic Case Management system

  3. Describe the importance of communication between the Case Management System Development Team members and their constituents during the planning and development process.
