Agility, the skill that enables case managers with experience and knowledge to identify the individualized care of complex patients. A key characteristic of the agile case manager is the ability to recognize the member’s needs and embrace a multidisciplinary approach to achieve a successful outcome. The agile case manager connects with the patient through outreach, listening, collaborating, and educating.
Member engagement is achieved by the case manager’s genuine use of empathy and compassion. The agile case manager inspires and educates by focusing on the instinctive nature of nurturing and tapping into learned skills and experience. Agile case managers reflect on their own faith to maintain the strength and integrity to balance the needs of their patients and themselves. These skills and characteristics of agility combine to empower the case manager to better maintain a balance in their professional, spiritual, and personal well being.
This poster will present inspirational wisdom from transplant case managers that will encourage others to utilize agility and perform beyond expectations professionally and personally. Their philosophy underscores the guiding principle of this poster: “shoot for the moon and should you miss, you will land among the stars."
Objectives:
- Applying this multidisci¬plinary approach daily will allow the case manager to perform beyond expec-tations to achieve a successful outcome.
- Incorporate learning tools and apply them into their profes¬sional and personal life. Measure success in the ability to balance their professional, spiritual, and personal well-being.
