This presentation will present the outcomes of the model presented in 2007. Initial data shows a significant increase in the patient's perception of quality health care and the physician's satisfaction in care for the difficult population of frail, older, adults. Discussions will cover interventions of the Guided Care Nurse and their relationship to the outcomes; the guided car nurse satisfaction with the role; and where guided care nurses go from here.
Additional outcomes data and methodology will be presented. The results of this trial and future plans and implications for wider dissemination will be presented; such as, the expected training needed to assume the guided care nurse role, the likely work setting and reimbursement model. The presentation will answer the questions: Is there evidence that Guided Care Nursing is Effective? and What are the Potential Opportunities for Case Managers if Guided Care Gains Widespread Adoption?
