Does hospital case management actually exist? This session will compare the most common models of hospital management with the formal definition of case management and Standards of Practice to answer this question. Then will utilize the same definition and Standards of Practice to construct the essential elements of a comprehensive hospital case management program that will produce better quality outcomes, decreased cost of care, fewer payor denials and more effective length of stay management.
Objectives:
- Compare various models of hospital case management.
- Describe Case Management Standards of Practice.
- Develop hospital case management program model that will produce quality cost effective outcomes with fewer payor denials and more efficient LOS management, while adhering to the Standards of Practice.
