Since outcomes of integrated care management focus on education, the question stands, “Does the patient hear or understand what the integrated care manager is saying?” Per the Case Management Standards of Practice, the first step is assessment.
This presentation will offer the cutting edge research on health literacy as well as varying assessments. In addition, the attendee will hear the controversy regarding screening health literacy. Resources will be offered with exciting solutions to plan for common issues found while educating the patient. This program will focus purely on the integrated care manager’s practice with solid take-home tools to assess, plan, intervene and evaluate the outcome of the education process.
