The National Health Service in England is undergoing a period of profound change which is impacting on clinical roles and standardization of care. The presentation explores mutual opportunities for learning by highlighting the differences between service models and delivery of care while acknowledging shared complexities and challenges. The Supportive and Palliative Care guidelines for best practice in England emerged in 2001 following the publication of the Cancer Plan in 2000.
Three, key, educational tools and resources subsequently emerged which have supported practitioners to develop new service models. These tools provided an underpinning evidence base for best practice and subsequently formed the basis for the current End of Life Care Programme.
By outlining the three recommended tools this presentation will show how this extensive work in the field of supportive and palliative care has now culminated in the imminent publication of the first definitive End of Life Care Strategy. It will explore how the role of a case manager in England, by incorporating established standards of practice, could add value within the provision of cancer and palliative care services and discuss how the role might help to achieve the required outcomes of the new strategy.
Objectives:
- Explain the ‘Supportive & Palliative Care Strategy in England—Cancer and Other Long-Term Conditions, which will allow delegates to apply their learning and explore how case management techniques can be applied to this strategy.
- Provide an explanation of the PICTOR tool that will allow them to demonstrate how elements of their own experience could relate to the Supportive & Palliative Care Strategy in England—Cancer and Other Long-Term Conditions.
- Discuss, with feedback, how to allow comparison of experiences to enable delegates to produce ideas for practical application of case management principles to the Supportive and Palliative Care Strategy in England—Cancer and Other Long-Term Conditions.
