Patients hospitalized at Stony Brook University Medical Center (SBUMC) for 30+ days accounted for over 10% of the daily census. Causes for the extended stay were varied and complicated. Administration established reducing extended length of stay as a strategic imperative. A framework to analyze processes and implement improvements in the extended length of stay patient set was established.
Through an interdisciplinary approach, the SBUMC Care Management Department engaged in a comprehensive, interdisciplinary, project to reduce not only the length of stay, but the number of patients in this group. In the span of just over one year, the aggregate patient days went from over 4,400 days to just over 3,000 days. This decrease had a major impact on decreasing the hospital's overall length of stay and resulted in a revenue opportunity of approximately $4 million.
Objectives:
- Identify the basic team structure.
- Identify three proactive measures to overcome roadblocks to discharge.
- Identify three proactive measures to communicate with treatment team.
