Planetree (www.Planetree.org) has been at the forefront of the effort to personalize, humanize, and demystify the health care system for more than 30 years, and this philosophy parallels that of case managers in many ways. As advocates for patients, case managers are personalizing, humanizing and demystifying the patient experience each and every day.
Founded in 1978 by a San Francisco patient, today Planetree is an internationally-recognized not-for-profit organization partnering with health care providers around the globe to advance patient-centered approaches to care.
Planetree-affiliated hospitals and continuing care communities are committed to the philosophy and implementation of the Planetree model of patient-centered care. Guided by the model's 10 core components, each member collaborates with Planetree staff to develop a plan to implement initiatives and practices designed to enhance the patient experience while also supporting the professional and personal aspirations of their staff.
Planetree shares practical, proven methods for the implementation of patient-centered care. It is a transformative experience that ultimately leads to more respectful, responsive care, and a great benefit to all health care consumers within an affiliate’s community. A key component is conducting patient focus groups to hear first-hand what is most important and meaningful to patients when they are in a hospital.
Planetree encourages non-restrictive visiting, processes for proactively sharing medical information with patients; and ongoing opportunities for patient and family input into hospital practices. Other initiatives include that staff participate in training to sensitize them to the patient experience, and that the hospital's physical environment is a healing one engaging all the senses.

Planetree's Annual Conference
