Led by an all star team of thought leaders, Dr. Eric Coleman launches Super Session I – focusing on models of care. Along with a team of distinguished co-presenters the Summit kicks off by outlining the conceptual framework surrounding new and innovative models. Notable in their respective arenas, Dr. Mary Naylor, Carol Levine and June Simmons will round out this presentation that will incorporate burning questions such as:
- The case for new models of care
- What makes new models unique
- Evidence that new models are effective
- Barriers to adoption and dissemination
- Impact on real issues in 2009 - aging, the uninsured, medication errors
- Developing, implementing, and evaluating the outcomes of new models

An equally noteworthy team kicks off Super Session II addressing the current hot topic of performance measures. In support of care coordination and transitions of care, performance measures are currently in development to move the field toward shared accountability across providers and systems. Chad Boult, Del Conyers, Anne Jones and Ed Davidson will sort through the issues and explore the tough questions like:
- Skill sets of clinicians to deliver care coordination and transitions of care
- Consumer demand for a higher quality of care
- Incorporation of patient values into performance measures across a multigenerational population
- How do we develop measures that are understandable from all vantage points, yet compelling
- The role of standardized patient assessments
- The role of information systems and tracking mechanisms
