MODELS OF CARE – SUPER SESSION I
 
Eric A. Coleman, MD, MPH, is Professor of Medicine within the Divisions of Health Care Policy and Research and Geriatric Medicine at the University of Colorado at Denver and Health Sciences Center. Dr. Coleman is the Director of the Care Transitions Program, aimed at improving quality and safety during times of care “hand-offs”. He is also the Executive Director of the Practice Change Fellows Program, designed to build leadership capacity among health care professionals who are responsible for geriatric programs and service lines. As a board-certified geriatrician, Dr. Coleman maintains direct patient care responsibility for older adults in ambulatory, acute, and subacute care settings.

Dr. Coleman's bridges innovation and practice through: (1) enhancing the role of patients and caregivers in improving the quality of their care transitions across acute and post-acute settings; (2) measuring quality of care transitions from the perspective of patients and caregivers; (3) implementing system-level practice improvement interventions and (4) using health information technology to promote safe and effective care transitions.

Carol Levine is the director of the United Hospital Fund’s Families and Health Care Project. She also directed The Orphan Project: Families and Children in the HIV Epidemic, which she founded in 1991. She was the director of the Citizens Commission on AIDS in New York City from 1987 to 1991. As a senior staff associate of The Hastings Center, she edited the Hastings Center Report.

In 1993 she was awarded the MacArthur Foundation Fellowship for her work in AIDS policy and ethics. Levine is the editor of Always on Call: When Illness Turns Families into Caregivers (2000) published by the United Hospital Fund.

Since 1990, Dr. Mary D. Naylor, PhD, RN has led an interdisciplinary program of research designed to improve the quality of care, decrease unnecessary hospitalizations and reduce health care costs for vulnerable community-based elders. To date, Dr. Naylor and her research team have completed three National Institutes of Health funded randomized clinical trials testing the Advanced Practice Nurse Transitional Care Model, an innovative approach to addressing the needs of high risk chronically- ill elders and their caregivers.

Dr. Naylor is also the National Program Director for the Robert Wood Johnson Foundation program, Interdisciplinary Nursing Quality Research Initiative (INQRI). In recognition of her research and leadership, Dr. Naylor has received numerous awards. In 2004, she was the first nurse selected as a McCann Scholar, the only national award by a private foundation that recognizes outstanding mentors in medicine, nursing, and science. In 2005, Dr. Naylor was elected to the National Academy of Sciences, Institute of Medicine.

Nationally recognized for her leadership in the administration of health care organizations, W. June Simmons is a visionary in developing innovative approaches to health care delivery in the 21st century. Throughout her distinguished career, she has been instrumental in creating, funding and operating forward-looking health and social services research and programs.

As founding President/CEO of Partners in Care Foundation, Simmons strongly believes that healthcare and social delivery of services need to change, especially in community and home settings. She takes an active role in developing initiatives and pro-active programs which meet the mutual needs of patient populations, providers, and health care delivery networks to encourage cost-effective, patient-friendly integration of care from hospital to home and community. Under her leadership, Partners has grown ten-fold since its establishment in 1998 and has been awarded major grants to investigate and evaluate new models of health care and social service delivery, including managed care environments.
 
PERFORMANCE MEASURES – SUPER SESSION II
 
Dr. Chad Boult is the Eugene and Mildred Professor of Health Policy and Management at the Johns Hopkins Bloomberg School of Public Health.

He directs the Roger C. Lipitz Center for Integrated Health Care and holds joint appointments on the faculties of the Johns Hopkins University Schools of Medicine and Nursing. A geriatrician for 20 years, he has extensive experience in developing, testing, evaluating, and diffusing new models of health care for older persons.

His current research includes Guided Care, a novel, multidisciplinary model of primary care for older people with multiple chronic conditions.

Del M. Conyers, MPH is a Senior Program Director with The National Quality Forum (NQF). The National Quality Forum (NQF) is a not-for-profit membership organization created to develop and implement a national strategy for health care quality measurement and reporting. A shared sense of urgency about the impact of health care quality on patient outcomes, workforce productivity, and health care costs prompted leaders in the public and private sectors to create the NQF as a mechanism to bring about national change.

Together, the organizational members of the NQF will work to promote a common approach to measuring health care quality and fostering system-wide capacity for quality improvement.

As a partner in Insight Therapeutics, a research and patient care consulting firm, H. Edward Davidson, PharmD, MPH is also an Assistant Professor at the Glennan Center for Geriatrics and Gerontology at Eastern VA Med School. In addition, he is Editor-in-Chief of THE CONSULTANT PHARMACIST, the journal of the American Society of Consultant Pharmacists representing pharmacists providing care to seniors in organized and other health care settings. Davidson also serves on the National Transitions of Care (NTOCC) Advisory Task Force.

Anne Jones is a Director at Humana, Inc., in Lousiville, Kentucky, a group health insurance organization committed to helping employers manage their health care costs, guiding consumers to make informed health and benefits decisions, and giving back to the communities we serve. Jones attended Sullivan University 2003 - 2004.
 
TECHNOLOGY – SUPER SESSION III
 
Dr. Bill Crounse, MD, is Healthcare Industry Director for the Microsoft Corporation. He is responsible for working with industry partners and health care organizations to help them benefit from using Microsoft technologies and solutions as an essential platform for line-of-business applications in the health care industry.

Dr. Crounse travels nationally and internationally to evangelize the use of information technology as a way to improve care quality and safety, and the satisfaction of those who provide and receive it.

Prior to joining Microsoft, Dr. Crounse was Vice President and Chief Medical Information Officer for Overlake Hospital Medical Center and the Overlake Venture Center in Bellevue, Wash.

He is also a founder of DoctorGoodwell.Net, a company that worked in partnership with Microsoft to improve connectivity between physicians and their patients. Before founding Doctor Goodwell (now doing business as Goodwell Technologies), Dr. Crounse was Vice President and CIO for Overlake Hospital Medical Center, a not-for-profit community hospital honored by Hospitals and Health Networks as one of the nations “most wired” and most innovative hospitals.

Dr. Crounse, a residency trained family physician, is a graduate of the Medical University of Ohio. Prior to joining Overlake Hospital in 1995, Dr. Crounse was a physician and medical executive with Virginia Mason Medical Center and Group Health in Seattle.

Adam Bosworth was until recently a Vice President of Product Management at Google Inc. He has started a new company, Keas Inc., that he says is “Dedicated to helping people stay as healthy as possible.”

He was previously senior VP Engineering and Chief Software Architect at BEA Systems and was responsible for the engineering efforts for BEA’s Framework Division. Prior to joining BEA, Bosworth co-founded Crossgain, a software development firm acquired by BEA in 2001.

Known as one of the pioneers of XML technology, Bosworth previously held various senior management positions at Microsoft, including General Manager of the WebData group, a team focused on defining and driving XML strategy. While at Microsoft, he was responsible for designing and delivering the Microsoft Access PC database product (code named ‘Cirrus’) and assembling and driving the team that developed Internet Explorer 4.0’s HTML engine (code named ‘Trident’).

Prior to Microsoft, Bosworth worked for Borland where he developed the Quattro spreadsheet application following Borland’s acquisition of Analytica in 1985 — founded by Bosworth and Eric Michelman, and managed by Brad Silverberg.

Bosworth received his bachelor’s degree in History from Harvard University in 1976.

Hugh Lee is President of Fusion Productions, a company that integrates design, production, and technology services for the meeting and incentives industry. Some of Lee’s industry involvements and accomplishments include:
  • Chairman of the Board, 2004-2005, Meeting Professionals International
  • Inductee to the 2002 Convention Industry Council Hall of Leaders
  • Inductee to the American Society of Association Executives’ Academy of Leaders
  • Member of the Executive Committee for Meeting Professionals International
  • Recipient of the 1996 Industry Award from Meeting Professionals International, given to those who elevate the meeting industry
  • Keynote at AsiaPacific Incentive & Meetings Expo in Melbourne, Australia, addressing the convergence of technology and its impact on the meeting industry

Donald Dea has a record of high-level success in the global technology, marketing, and business development areas. He has an MBA from the Fuqua School of Business, Duke University, and graduated from MIT’s distinguished Sloan School of Management’s Senior Executive Program. Don served as Special Assistant to the Attorney General in the President’s Executive Exchange Program.

Mr. Dea's career includes:
  • General Manager of U.S. Customer Operations for the Xerox Corporation, responsible for digital channel sales and marketing and for developing strategic partnerships with groups such as EDS, Novell, Scitex, and Arthur Andersen.
  • He also served as Xerox’s General Manager of OEM/VAR U.S. Channels.
  • As co-founder of Alaris Corporation, he developed a family of 486 and Pentium PCs, and established a global distribution network that included Europe, U.S., South America, Pacific Rim, and the Middle East. Alaris grew from start up to over $90 million in sales in three years.
