 Effective collaboration among members of the care coordination team is essential in the current managed care environment to meet the increasingly complex needs of patients and families. Data in the scientific literature suggests that the model and level of collaboration between the patient's physician(s) and the patient's case manager(s) can positively or negatively influence appropriate access to health services, cost and quality of care. The Case Management Society of America Standards of Practice for Case Management cites physician/case manager collaboration as an essential philosophical component of the case management process.
With these facts in mind, an expert panel was convened on April 4, 2003, of medical directors and directors of case management from leading health care organizations across the nation. Their goal was to reach a consensus on effective physician and case management collaboration to improve patient care, including identifying barriers and solutions to achieve effective collaboration. Case studies exploring best practices in physician and case management collaboration were submitted and reviewed by the expert panel. The panel also reviewed intellectual and scientific data in preparation for facilitated dialogue. A framework to establish and improve collaboration between physicians and case managers was achieved. David B. Nash, MD, MBA, FACP, of Jefferson Medical College, Thomas Jefferson University, and Kathleen Moreo, RN, Cm, BSN, BHSA, CCM, CDMS, of P.R.I.M.E., Inc. co-chaired the meeting.
