1. Sponsor.
   CMSA Monthly Giveaway ("Sweepstakes") is sponsored by Case Management Society of America, 6301 Ranch Drive, Little Rock, AR 72223 ("CMSA" or "Sponsor").
    
2. Sweepstakes Period.
   The CMSA Monthly Giveaway corresponds to a particular month and is accessible from www.cmsa.org/conference. For applicable sweepstakes periods and entry deadlines, see specific CMSA Monthly Giveaway dates on www.cmsa.org/conference.

3. To Enter.
   No purchase necessary to enter or win. Registrants for CMSA’s 20th Annual Conference and Expo will automatically be entered for all remaining monthly giveaways from the time their registration is received by CMSA. Alternatively, to enter by mail, send a 3"x5" card hand-printed with your full name, date of birth, postal address (including street, apartment/suite number, city, state and zip code), phone number and e-mail address (if any) to: CMSA Monthly Giveaway, Case Management Society of America, 6301 Ranch Drive, Little Rock, AR 72223. Limit one (1) entry per outer mailing envelope and per mailing address. Mail-in entries will be good for one drawing and will entered for the monthly drawing following that follows receipt of the entry card. Any mail-in entry that is illegible, incomplete, late, or does not carry proper postage will be disqualified. Only one entry per day is allowed. VOID WHERE PROHIBITED. Automated or computer generated entries or mechanical reproductions are prohibited, and any use of automated devices or mechanical reproductions will cause disqualification. The end date of the Sweepstakes Periods and the prizes are listed in the CMSA Monthly Giveaway section on the CMSA website (www.cmsa.org). LIMIT: ONE ENTRY (PER SWEEPSTAKES) PER PERSON, HOUSEHOLD, FAMILY AND/OR E-MAIL ADDRESS. Sponsor and its advertising and promotions agencies are not responsible for lost, late, incomplete, illegible, mutilated, inaccurate, misdirected, stolen or postage due entries or mail, or for any problems of any kind whether mechanical, human or electronic, including, but not limited to, computer, telephone, cable, network, satellite, electronic or Internet hardware or software malfunctions, failures, connections, availability or garbled or jumbled transmissions, or service provider/Internet/web site/use net accessibility or availability, traffic congestion, or unauthorized human intervention. Sponsor reserves the right to suspend or terminate any sweepstakes in the event of any such problem. Sweepstakes entries are the property of Sponsor. E-MAIL CHANGE OF ADDRESS POLICY: It is your sole responsibility to notify Sponsor in writing if you change your e-mail address. (To do so: write to: cmsa@cmsa.org).
    
4. Prizes.
   The amount or other description of each prize is stated in the CMSA Monthly Giveaway section. Stated prizes will be awarded to all eligible winners of each Sweepstakes who claim the prizes within 30 days of notification to the winner by Sponsor. Prizes not claimed within such 30-day period may not be awarded in the sole discretion of CMSA. Federal, state and local taxes are the responsibility of the winner. If by reason of any error (relating to electronic transmission, technical or human errors or otherwise), more prizes are claimed than the number set forth in the applicable CMSA Monthly Giveaway, all persons making purportedly valid claims will be included in a random drawing for the number of prizes as stated. Individual prizes will be subject to varying terms of redemption. All prizes connected to CMSA’s 2010 Annual Meeting must be used between June 8 and June 11, 2010. All other prizes must be redeemed by June 30, 2010. No prizes may be redeemed for cash value.
    
5. Drawing; Odds of Winning:
   Prize winners will be selected in a random drawing from all entries received by the applicable deadline. Drawings will be held no later than 90 days after the deadline posted on the CMSA Monthly Giveaway. There will be a separate drawing for each Sweepstakes. For each Sweepstakes, odds of winning will depend upon the number of entries received.
    
6. Awarding of Prizes.
   Winners of the Sweepstakes will be notified by e-mail (the "Notification") within 15 days of the drawing. Winners without an email address will be notified via postal mail at the address provided in their entries. Winners will be required to complete a registration form, which will include an online Affidavit of Eligibility and, where lawful, a Liability/Publicity Release. By completing the affidavit and release, the winners grant permission to the use of their names and addresses for advertising and promotional purposes for this and similar promotions without additional compensation unless required by law. Prizes will be awarded within 30 days after receipt of all online registration forms. Prizes are not transferable. Non-compliance with these Official Rules or Terms of Service or undeliverable notification may result in disqualification and selection of an alternate winner.
    
7. Eligibility.
   VOID WHERE PROHIBITED. To enter, you must be over 18 years old at date of entry. Employees of CMSA, its affiliates, subsidiaries, advertising and promotion agencies, manufacturers or distributors of sweepstakes materials, and their respective officers, directors, shareholders, agents, representatives and members of their immediate family are not eligible. All applicable federal, state and local laws and regulations apply.
    
8. Conditions.
   1. All CMSA Monthly Giveaway Sweepstakes are governed by these Official Rules. By participating, entrants agree to these Official Rules and understand that all decisions regarding the Sweepstakes including, but not limited to, winner selection, will be made by Sponsor, whose decisions shall be final and binding in all respects. Sponsor is not responsible for (i) lost, late, delayed, misdirected, unintelligible, incomplete or postage due mail or electronic mail; (ii) telephone, electronic, hardware or software program, network, Internet or computer malfunctions, failures, or difficulties; (iii) errors in transmission; (iv) any condition caused by events beyond the control of the Sponsor which may cause the Sweepstakes to be disrupted or misused; or (v) any inaccurate information caused by any reason, including, without limitation, the cmsa.org website, users, equipment, or human or technical errors relating to the submission of entries.
   2. By participating in the Sweepstakes, you agree to release and hold harmless Sponsor, its clients, and promotional and advertising agencies, the entities whose names or brands appear in any CMSA Monthly Giveaway, and all of their respective officers, directors, shareholders, employees, affiliates, representatives and agents from any and all losses, damages, rights, claims, actions and proceedings of any kind arising out of or relating to the Sweepstakes or resulting from acceptance, possession, use, or misuse of any prize, including, without limitation, personal injuries, death, and property damage, and any claims based on publicity rights, defamation, or invasion of privacy. IN NO EVENT WILL SPONSOR, THE ENTITIES WHOSE NAMES OR BRANDS APPEAR IN ANY CMSA MONTHLY GIVEAWAY, OR ANY OF THEIR RESPECTIVE OFFICERS, DIRECTORS, SHAREHOLDERS, EMPLOYEES, AFFILIATES, REPRESENTATIVES AND AGENTS BE RESPONSIBLE FOR DAMAGES OR LOSSES OF ANY KIND, INCLUDING, WITHOUT LIMITATION, DIRECT, INDIRECT, SPECIAL, CONSEQUENTIAL OR PUNITIVE DAMAGES, ARISING OUT OF OR RELATING TO YOUR USE OF THE CMSA.ORG WEB SITE OR THE DOWNLOADING FROM AND/OR PRINTING MATERIAL DOWNLOADED FROM THE CMSA MIX WEB SITE. WITHOUT LIMITING THE FOREGOING, EVERYTHING ON THIS SITE IS OFFERED "AS IS" WITHOUT WARRANTY OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING FITNESS FOR A PARTICULAR PURPOSE OR NON-INFRINGEMENT. SOME JURISDICTIONS MAY NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES OR EXCLUSION OF IMPLIED WARRANTIES SO SOME OF THE ABOVE LIMITATIONS MAY NOT APPLY TO YOU.    3. Any and all disputes, claims and causes of action arising out of or connected with the Sweepstakes, including, without limitation, relating to any prizes awarded or not awarded, shall be resolved individually, without resort to any form of class action, and exclusively by binding arbitration in Chicago, Illinois, by a single arbitrator in accordance with the Commercial Arbitration Rules of the American Arbitration Association ("AAA"). Any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering the Sweepstakes, but in no event attorneys' fees.
   4. The Sweepstakes may be modified, canceled, terminated or suspended in whole or in part without prior notice for any reason, including, without limitation, computer virus, bugs, non-authorized human intervention, mechanical or electronic problems or other causes beyond the control of Sponsor which corrupt or impair the administration, security, fairness or proper play of the Sweepstakes. In the event of early termination of the Sweepstakes, all prizes will be awarded in a random drawing from among all eligible entries received through the date on which the Sweepstakes is terminated to the extent practicable, as determined by Sponsor in its sole discretion.
   5. Offer void where prohibited and subject to all applicable federal, state and local laws.
