Transitions are a natural occurrence in military life, and military (DoD/VA) healthcare is no exception. DoD/VA case managers assist patients through the complexity and maze of the DoD/VA healthcare system as they travel the healthcare continuum. The presentations in this conference will not only provide the case manager’s point of view, but also that of the patient and family. Some of the toughest transitions that case managers face will be addressed to include: considering the multiple needs of the seriously ill or injured warrior; dealing with pain manage­ment; the unique needs of the female warrior; addressing special needs of family members; coordinating mental health services; and navigating the MEB/PEB process. This "DoD/VA Case Management 2010 Conference:  Seamless Transitions Across the DoD/VA Healthcare Continuum” presents not only the multiple challenges/barriers/obstacles DoD/VA case managers face, but also offers suggestions to overcome these challenges/ barriers/obstacles.
