Chronic obstructive pulmonary disease (COPD) accounts for nearly 43 billion dollars per year in direct health care expenditures and indirect costs due to morbidity and mortality. (NCQA, 2008)  Case managers have a role to play in educating patients, and helping patients find the motivation and self management skills to better manage their disease.

This symposium of the COPD Case Management Adherence Guidelines will review the latest management and treatment recommendations and Case Management interventions to assist patients living with COPD.

1. Provide a definition of COPD.
2. List the characteristic signs and symptoms of COPD.
3. Describe the four components of COPD Management.
4. Identify key adherence challenges and possible case management interventions.
5. Identify case management strategies in different practice settings and recognize components of a transition of care plan.
