Today’s Case Managers work more intensely with patients and their family caregiver addressing medically complex issues. Identifying atrial fibrillation and addressing the case management treatment options with patients involves the multiple interventions including disease management, medication adherence, care coordination and transition of care.
This symposium will help the case manager with understanding the options and developing care planning initiatives.
Objectives:
- Understand Atrial Fibrillation (AF) – the basics of etiology, pathology, and prevalence.
- Identify evidence-based guidelines that for AF.
- Discuss case management initiatives that promote disease management and patient safety through each transition of care.
