Objectives:
  1. Identify key components of the Clinically Integrated Delivery Model (CIDM). (People and process combined with a clinical platform with enhanced capabilities and tools.)
  2. Describe the benefits of enhancing nursing skills combined with a designated specialist team to deliver an integrated approach to care management.
  3. Recognize the importance of understanding the client's culture and integrating a culture of health among Employees.
  4. Describe the use of technology solutions to monitor populations, and synchronize data in order to proactively identify and prioritize opportunities to deliver a holistic approach to care management.
  5. Understand the impact that CIDM will have for empowering the patient's healthcare decisions in collaboration with their referring physicians, providers and case manager.
