The United States Air Force Nurse Corps is pleased to announce its annual Health Care Integrator (HCI) Conference.  This forum provides the USAF HCIs and their colleagues a forum to network and learn about topics that improve their ability to provide Population Health and Medical Management services to their beneficiary population.  Partnering with CMSA also provides an excellent opportunity to network with their civilian colleagues and learn about the latest topics in Case Management.

This year’s theme will be Population Health through Synergized Partnerships.  The combined efforts of HCIs, Medical Management Staff, the Air Force Medical Operations Agency (AFMOA), the Air Force Medical Support Agency (AFMSA), and the AF Clinical Informatics Branch will only serve to achieve the best patient outcomes. The topics will focus on updates from key partnerships that assist the HCIs in their daily operations. This session is open to all USAF HCIs and staff working for the Department of Defense.
