As hospitals and acute-care settings self-police their use of clinical resources, whether at the bedside or system-wide, case managers will play a vitally important role. Case managers, working in concert with physicians, nurses, and others, will help spearhead the drive toward more efficient allocation of resources to meet the needs of patients, support successful transitions, avoid unnecessary emergency department visits, and readmissions in the future.
Far beyond utilization review, resource management is a critical element in advocating for patients in the most efficient and effective way, while also achieving favorable outcomes for the institution. One way to promote resource management, as part of practice decisions, is to involve case managers with patient rounds, along with nurses and other clinicians. This enables the case manager to assist, raise questions, and work closely with nurses to ensure the patient is receiving the right care and treatment, without unnecessary testing or duplicative services.
This presentation will be made by two nurse case managers with extensive experience in hospitals, acute-care settings, and resource management, who are also Commissioners with the Commission for Case Manager Certification (CCMC). Their program will focus on the following points:
1) The importance of resource management.
2) The role of the case manager in resource management.
3) Case managers and the physician-based model.
4) Resource management as a support for successful transitions of care.
This informative program will be of interest to all case managers, and not just those who work in a hospital or acute-care setting. Case managers involved in other venues along the health care spectrum—from home health to rehabilitation—will benefit from the discussion of clinical resource management. The program will demonstrate how case managers are uniquely positioned to ensure that goals and outcomes of care are achieved in a cost-effective manner. By integrating and monitoring resources, they strive to achieve the maximum benefit while eliminating waste and fiscal irresponsibility that drive up the cost of care.
