The role of the case manager is transitioning. An increase in patient complexity, changes in regulatory compliance, and challenges of the unfunded and underfunded patient--all impact the case manager’s ability to coordinate care in a timely manner with optimal outcomes. This session will address the challenges of both the case manager and the case management leader in the evolution of hospital case management. This session will also provide updates on issues of concern, including discharge planning and readmissions, keeping your case management department compliant, monitoring your case management outcomes, and using data to support process improvement.  


Objectives:
  1. Develop strategies for case management practice.
  2. Establish optimal outcomes management monitoring using case management “report cards.”
  3. Identify upcoming challenges facing case managers and case management leaders.
