Cheryl A. Acres, RN, CCM--As a frontline case manager, supervisor, and district manager, has over 15 years’ experience in Case Management, Independent Case Management practice, Medicare Home Health Care, Worker’s Compensation, and Self-Insured/Indemnity Plans, as well as LTAC. 

She developed an independent case management company focused on the Geriatric Market in the private sector, as well as consultative services. Cheryl has also demonstrated high-level outcomes for her multiple clients through the years, with a focus on ethical and quality services.

Specifically, she has had extensive experience working in home health, as well as LTAC. Cheryl has also coordinated care through the various levels of post-acute care (e.g.; LTAC, acute rehab, skilled nursing facilities, home health care, home infusion, and post-acute brain injury programs).  She was the nurse educator for home healthcare, and wrote the competencies for all disciplines within the agency. 

Cheryl has been a business owner, practicing independent case management for the private sector with a focus on elder care and services available in the community.  She has developed an extensive network of service providers and contacts related to the seniors, including other geriatric care managers. She has also developed marketing tools and resources for her business, as well as becoming a Steering Committee member for the Senior Care Committee for the Health Industry Council in the DFW market.
